The investment for this elegant evening is $100 per ticket.
When you buy two tickets, you will receive a third ticket to share the fun with a friend.

Note:  Person #1 is the person completing this form.

Person #3:  If you purchased 2 tickets, you have two options:

1.  Bring a Friend
     You may invite someone to join you!  Please provide their
     name so the Palooza Team can contact them regarding their
     food preferences.

2.  Choose "GIFT"
     If you'd prefer to donate the third ticket, select "GIFT" and 
     the Palooza Team will offer it to someone who would love to 
     attend but may not otherwise have the opportunity.

This event is catered by Vitos Italian Ristorante, and we want each guest to enjoy a meal that suits their needs and preferences.
